Financial Hardship
For any FZS family experiencing a financial hardship that may impact their child's ability to participate in an FZSBHC season.
We recognize the sensitive nature of these circumstances and their need to be handled with care. As such, these requests and/ or subsequent communications will be treated as strictly confidential.
Ice Hockey
Medical Consent Form
Each player must have a Definitive Emergency Medical Care Consent on file that is updated each season. Submit the completed and signed form to the FZSBHC Registrar.
Academic Eligibility
All Varsity/B-Team players need to have their previous school year's report card or transcript uploaded prior to the start of ice practice. Mid-States requires that all high school players have passed at least three credits of classes in the most recently completed semester.
Inline Hockey
Academic Eligibility
All players must have their most recent report card on file with the FZSBHC Registrar.
